Financial Information:

For your convenience, we accept Visa, MasterCard, American Express and Discover in addition to cash or check. Drs. Cummings, Balasanian, and Caputo, provide the optimal care at the most reasonable cost to our patients. Payment is due at the time service is rendered unless other arrangements have been made in advance.

Patients will receive a monthly statement for any outstanding balances. Most insurance companies will respond within four to six weeks. Any remaining balance after the insurance payment has been received is the responsibility of the patient. Your prompt remittance is appreciated.
